This form is used to obtain general information from an applicant that is appealing to the Board of Adjustment for a Conditional Use Permit.
The form should be completed and signed by the applicant. It is submitted to the Division of Planning along with all of the other required application materials, as detailed in (link to general instructions). It should only be completed after obtaining a “refusal form” from the Division of Building Inspection, to ensure that the correct application form is used and the nature of the appeal is correctly identified.
